New Patient Registrationregister_now

We ask all new patients to fill in a registration form and to complete a new patient questionnaire. Both of these forms are available to download from the above "Registering Online" tab.

It is important that you provide us with your previous doctor’s name and address. A practice leaflet is provided which includes all the details about the surgery and service we provide. Once we receive the completed forms your registration details will then be added to our computer system, enabling you to contact the surgery and book appointments. If you complete the forms online you will need to attend the surgery to sign these form before being registered at the surgery.

Medical treatment is available from the date of registration. If you are on any repeat medication you will need to be seen by the GP in a routine appointment before your medication will be issued. Please contact reception for further information.

Named GP

From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP. This GP has responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required, based on the GP’s clinical judgement.

This does not mean that you will only see your named GP, you will continue to be able to see any member of the clinical team and your care will still be provided by the most appropriate member of the clinical team here at Park Lane Surgery.

If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.  Please inform us if you wish to change your accountable GP.
